Your First Visit
During your first visit to Dr. Carr Orthodontics, you’ll be asked to complete your new patient paperwork if you were unable to do so prior. Then you’ll meet Dr. Carr’s treatment coordinator who will take x rays and photos that Dr. Carr will review to determine the best orthodontic treatment for you.
Patient Forms
When you become a new patient at Carr Orthodontics, we will add your information to our patient management system. You will then receive an email and a text message with a link to create your own account and fill out all necessary paperwork online.
